Cómo trabajamos juntos

The continuity-of-care workflow.

Five steps. The veterinarian remains at the start and the end of every loop.

  1. 01

    Veterinary Hospital

Your diagnosis, your discharge plan, your prescription, your follow-up timing.

  2. 02

    Xavier's Veterinary Care

In-home visit. We reinforce your instructions, support the owner, and assist with technologist-scope care.

  3. 03

    Home Education & Support

Hands-on coaching, calm environment, owner questions answered, fear-free handling.

  4. 04

    Visit Summary

Written documentation delivered to the owner, and with consent to your hospital, within 24 hours.

  5. 05

    Primary Veterinarian Follow-Up

New observations, owner questions, and any concern beyond our scope route back to your team.

How we extend your care

Eight services. One promise: your team stays at the center.

XVC is your extended-care partner — never your competitor. We do not diagnose, prescribe, perform surgery, or displace the veterinarian-client-patient relationship. Every visit reinforces your treatment plan, documents what we observe, and routes every new clinical concern straight back to you.

Discharge support

We sit with the owner and walk through your discharge plan, step by step, in the calm of home.

Compliance coaching

En lenguaje sencillo education built around your treatment plan. Higher adherence, fewer callbacks to your front desk.

Home observation reports

Structured notes on behavior, appetite, mobility, and environment — delivered to your chart with consent.

Medication support

Under your direction. We coach owners through oral, transdermal, injectable meds, and sub-Q fluids.

Senior pet support

Mobility, environment, and quality-of-life visits — coordinated with your geriatric care plan.

Hospice support

Comfort visits coordinated with the attending vet. Euthanasia decisions remain entirely with you.

Wildlife referral

Owners with found wildlife receive accurate NYC rehabilitator referrals — never sent to your clinic.

Direct communication

Visit summaries delivered in your preferred format — PDF, SOAP, email, or fax. Chart stays current.

Education extends past the visit

Gratis learning for your clients and staff.

XVC maintains a 350+ guide educational library — pet health, preventive care, breed-specific guías, wildlife, One Health, emerging diseases, diagnostic learning, and more. All free to share with the families and team members in your practice.

For your staff

The Diagnostic Learning, Clinical Guides, Vet Career Pathways, and Emerging Diseases sections are written for techs, students, and curious clinicians. Vida silvestre y One Health covers the broader public-health context.

Donation-based educational mission. XVC's library is free to read and share. Consultation referrals from your practice may carry a small professional consultation deposit depending on scope. We work transparently. All standards and policies live here.

Scope of practice

What XVC will never do.

These are not constraints we apologize for. They are the foundation of why your team can trust us.

Diagnose disease

We do not interpret labs, imaging, or symptoms as a diagnosis. All diagnostic decisions belong to the veterinarian.

Prescribe medications

We do not initiate, change, or discontinue prescriptions. We support administration of medications you have prescribed.

Perform surgery or invasive procedures

All surgical and procedural care remains entirely within the veterinary hospital, where it belongs.

Replace the veterinarian-client-patient relationship

We exist to strengthen the VCPR — not establish a parallel one. Every interaction reinforces the client's relationship with their primary vet.

Referral Program

For clinics ready to extend care into the home.

Clinics, hospitals, shelters, and rescue organizations may refer clients to XVC for in-home education and support services. The process is confidential, simple, and respects your existing care plan.

Step 1

You identify a candidate client

A senior patient needing home support. A post-op owner overwhelmed by aftercare. A hospice case the family wants to manage at home. A new puppy household with too many questions.

Step 2

You complete a brief referral

A short form (currently being developed — see below) captures patient details, your care plan, and what you'd like XVC to reinforce. We confirm receipt within one business day.

Step 3

We coordinate & report back

XVC contacts the client, schedules the visit, executes within scope, and returns a documented summary to your hospital. The loop is closed.
